Job Summary: Provides high-quality, patient-centered care in the home setting. Flexible scheduling and manageable caseloads that prioritize outcomes. Wanting a personal, respectful, evidence-based therapist that empowers patients to remain safe and independent at home.

Responsibilities

• Perform skilled PT evaluations and treatments in patient homes
• Develop individualized care plans based on patient goals
• Educate patients/caregivers on safety, mobility, and home exercise programs
• Complete timely, accurate documentation (including OASIS when applicable)
• Collaborate with the interdisciplinary team for care coordination
• Maintain professional, compassionate communication and clinical quality

Qualifications

• Licensed Physical Therapist (PT)
• Home health/geriatric experience preferred (not required)
• Strong clinical skills and commitment to patient-centered care
• Reliable transportation
